Duplicate License
To obtain a duplicate of your license, please submit the following:
a letter of request which includes-
your name
your folder number or social security number
your address
a fee of $15 made payable to the Board of Educational Examiners
Submit the information to:
Board of Educational Examiners
Licensure Bureau
Grimes State Office Building

Des Moines, IA 50319
You may also renew your license with your credit card at our website, www.boee.iowa.gov
