Feb. 28, 2011 - A revised Certificate of Death form was implemented by the Iowa Department of Public Health, Bureau of Vital Statistics, effective with deaths occurring at 0000, January 1, 2011. The Department of Inspections and Appeals (DIA) is making this information available to long-term care facilities and other healthcare providers as a courtesy.
