Reporting a Complaint
If you feel that the nursing care provided has not been satisfactory in some way, you should report your concerns to the board's enforcement unit. The situation will be investigated in order to determine if any rules or laws appear to have been violated. The information you provide is helpful in making certain we are able to investigate the matter without any unnecessary delay. In your complaint, answer as many of the following questions as possible:
1. What is the nurse's name?
2. Where does the nurse work?
3. What happened?
4. When did this occur (date/time)?
5. Who witnessed this?
Additional helpful information:
The nurse's license number
The nurse's current home address
The nurse's current employer, if it has changed
