Name changes are not available online.

Licensees are required to notify the board office of any change in their name or address, IAC 655 3.7(1). To notify the board of changes choose one of the following:

Please include your license number, previous name/home address, and current name/ home address. Employment address is not acceptable.

IF YOU ARE AN LPN IN IOWA AND HAVE APPLIED FOR THE RN EXAMINATION, YOU WILL NEED TO DIRECT YOUR CHANGE OF ADDRESS TO THE NEW LICENSURE DEPARTMENT: Email Change of Address, SO THAT BOTH RECORDS ARE UPDATED. WHEN SUBMITTING CHANGES, PLEASE INCLUDE YOUR LPN LICENSE NUMBER OR SOCIAL SECURITY NUMBER FOR IDENTIFICATION PURPOSES.

Name Changes:

Licensees who notify the board of a name change will not automatically receive a license card in the new name. If a license in the new name is desired, there is a fee of $20.00 for a re-issued/duplicate license. If you would like a re-issued wallet card, send a written request and the current wallet card including the $20.00 fee. If you do not have in your possession a current wallet card you will need to complete the request for duplicate wallet card form, Application for Duplicate Wallet Card or Certificate which will need to be notarized and mailed to this office along with the $20.00 fee. The Administrative Rules require that all persons holding an active license have in their possession a current wallet card; however, the rules do not require that a licensee obtain a re-issued card if the name or address changes during the licensure period.
