4. Mail or FAX the application to the Iowa Board of Nursing – FAX Number: 515/281-4825

This method will retrieve the identical data as the On-Line License Verification using a license number.
A login I.D. and password will be sent to the email address provided; you will then be allowed to enter the secured site.
